my info might be wrong unless the plane was in more than one crash. The incident report at Joliet Illinois for that tail number was in July of 1964.

Student jumper landing next to the runway, student pilot landing on the runway panicked and tried to go around, flew into canopy.  
Jumper had broken pelvis and other injuries.
